To facilitate being seen just as soon as possible at the time of your appointment, we would appreciate it if you would complete the Patient Registration Form and HIPPA Consent Form before your arrival. The HIPPA Policy Form can be reviewed before you sign the HIPPA Consent Form.

Please remember to bring both forms with you at the time of your appointment or mail the completed forms back to us as soon as possible. We will also need any Insurance cards that might apply to you or your family when you come in so we can include your insurance information with your records.
